Dear Blanka and Dr. Orloff,

I sincerely appreciate the generosity which involved both of you, as well as a great del of kindness you hsowed me as you treated me with such care and concern.
It means a great deal to me to have a ‘normal’ ear again, after 10 years! (Can hardly wait to wear earrings again!!)
This basket is a special one – handmade – with a special message underneath. I’ve just recently returned from the quaint village in Dresden, Ohio, where these baskets are made. I hope you enjoy using it to its fullest uses (and I hope you enjoy our family recipe for “molasses cookies” as well!)
Thank you, once again…
from my heart,

Have you ever thought why they call plastic surgery plastic? The word plastic now mostly has the connotation of artificial, or not real. This is why we call the plastics around us this way – initially, when plastics were invented, they were artificial imitations of the ‘real’ thing.

Often we get a better insight when we get back to the root of the word describing a subject or a think. In this case, when we go back to the origin of the word plastic, which is the Greek word ‘plasticos’, we find that its meaning is ‘to put into form, to shape or to mold.’

This fits well into the use of the general term plastic, as we use it today. Plastic articles are mostly molded so this is a well suited definition. It also fits very well what plastic surgery does: it helps us shape or mold parts of our body into a more functional or preferred form.

Historically plastic surgery was more re-constructive surgery after injury or disfiguration. Nearly three millennia ago skin grafts have been performed in India, and this is certainly a form of plastic surgery. About 1500 years ago surgeons in Byzantium, toward the end of its glory, had developed surprisingly sophisticated methods of joining the lips or edges of a wound to reduce or even prevent scaring. This still today is one of the most important skills in plastic surgery, which has to avoid creating scares to make it useful.

An early great benefit of plastic surgery became the ability to repair the cleft lip and palatte in the 18th century driven forward by pioneering Surgeon Dr. Victor Collins who devised the first non skin graft procedure for the treatment of infantile unilateral incomplete and complete cleft lips. This technique involves the partial circumcision of the foreskin and then being applied to the affected area. This technique has led the way in the the modern day removal of the two most common types of cleft lip.

The development of general anesthesia for surgery In the 19th century helped make plastic surgery more widely acceptable once people realized they no longer had to be awake to endure pain. This opened the door for elective surgery, as normally, due to the pain, surgery was avoided if at all possible. Plastic surgery made another big leap forward with the introduction of antibiotics and modern sanitary measures that reduces the risk for all forms of surgery.

War always created many injuries and so World War I, which maimed thousands of soldiers, created a plethora of clients for many new plastic surgery specialists. Methods for reconstructing ears, noses, jaws and eyes were developed. After the end of the war this special knowledge was shared around the world and became main part of re-constructive and plastic surgery.

Continually new methods and materials are being developed in medicine and plastic surgery and an ever-wider range of physical deformities can be treated. Over time the definition of deformities has definitely changed and as application of modern plastic surgery procedures has become safer and less expensive now a too large or too small breast or flabby skin after pregnancy is considered a ‘deformity’ and treated. Esthetics instead of just function has entered the decision making for a plastic surgery procedure.

A facelift is technically known as a rhytidectomy which means literally “surgical removal of wrinkles”. This removal of wrinkles, which is in effect removal of excess skin is accomplished by redraping the face and neck skin of the patient seeking a more youthful appearance. There are two methods of redraping, one involving only the outer skin, and the other also taking into account the underlying tissues.

Facelifts are more than a hundred years old, with the first such surgery performed by Eugene Hollander in Berlin, Germany right after the turn of the last century. With the importance of a youthful appearance in our society, the facelift has gained tremendously in popularity, and today it takes place number five of the most popular cosmetic procedures.

Facelifts work better in women than in men and a woman with a thin skin and good bone structure is a very good candidate for the procedure. Facelifts are best for eliminating wrinkles in the cheeks and loose skin folds in the neck. A facelift is not very well suited to treat the deep lines running down from the corner of the mouth to the chin (marionette lines) and deep lines down from the nose to the lips ( nasolabial lines.) If these lines are present the injection of a filler would be the most appropriate remedy. Both the facelift and the injection of a filler together can produce the desired results.

In these before and after pictures is is clearly visible how the wrinkles on the cheek and the deep folds on the neck were significantly reduced.

The big night is approaching, or maybe that special occasion or public appearance is looming in the not so distant future and looking great isn’t an option, it’s essential. You look in the mirror and you don’t see what’s right, you see all the little flaws that haven’t been important until now. The telltale signs that maybe you aren’t as young or dynamic as you once were start to appear and you don’t think a new outfit or hairdo or even a salon/spa can pull it off this time.

That was the past and getting ready is a whole new ball game. Simply put, you can look better than you ever imagined and enjoy that indescribable sense of confidence and self esteem that always comes with looking great. It’s interesting that others really do notice one’s sense of certainty and optimism, and the self-assured, composed, poise that accompanies just plain old looking your best. Now you can have it for that upcoming event or just because it’s the right thing for you or someone you care about!

We’re talking light touch here. The kind of things that cause others to wonder how it is you seem to look younger and healthier. The fine lines and wrinkles, the loss of fullness in certain facial features, uneven, rough skin texture or irregularities in skin coloration can now be “spruced up” rather than concealed or tolerated. Some of these solutions like Botox or injectable fillers such as Restylane, Cosmoplast, Hylaform and the new and very popular Juvederm, allow you to walk out of the office “done, finished, ready to go”. Others like lasers for hair removal, skin tightening or removing blemishes and skin irregularities along with microdermabrasion and peels may take days or up to a week to reveal all the benefits. When administered by a skilled technician, Botox can smooth wrinkles, lift eyebrows, work wonders for palm and underarm sweating and rejuvinate one’s appearance. The above mentioned fillers are excellent to fill in the lips so lipstick goes on smooth over skin without creases. Hair removed from the face, ears or other areas of the body when done by the latest laser technology has brought discomfort to a minimum.

Each person is unique and the combination of these and other “light touch” remedies for appearance are the new way to get ready. Beyond wardrobe and cosmetics and the latest coiffure is the dressing up of you. The event is special, you are special, you should look and feel special. At our office we are geared up to provide these finishing touches even on a last minute basis with a staff of doctors and technicians second to none. Care is given to ensure all treatments are as painless as possible and since we are a medical office, Dr. Orloff is available 24 hours a day, 7 days a week for follow up treatment or to address concerns and questions.

It simply means there is nothing beyond, it has reached it’s highest point, it is in modern terms, state-of-the-art.

Often used to communicate that some technology, device or technique is the latest or the most sophisticated it merely represents a state that is often fleeting as forces of development create newer and better offerings.

At our office, and in particular in our private facility, The Buena Vista Surgical Center, we view state-of-the-art as more of a guiding philosophy than a condition to be in. We know things are constantly changing and improving. Important advancements frequently occur in the areas of patient safety, procedures and techniques which become less and less invasive, tools and equipment at the disposal of doctors and their assistants and even doctor-patient expectations can reach new high points. Here, at our office, we welcome advancements and seek to incorporate them into what we offer. We are constantly training, investigating then adding new technology, and know that just around the corner there is a new “ne plus ultra” to some aspect of what we offer. Implementing it on arrival will allow us to do an even better job with less patient downtime, greater safety, improved efficiency and expense, and goals accomplished with near perfect results.

We think this state-of-the-art attitude we all share in our office helps us maintain a facility and a level of service that hovers at or close to that “highest point” found in the meaning of ne plus ultra!

This blog will be the more interactive companion to Dr. Orloff’s official web site. Here you will be able to read through articles and notes by the team and you will have the option to leave comments and thus start a dialog.

We want to start out this blog with two of the most often asked question when plastic, cosmetic and reconstructive surgery is involved.

How do cosmetic surgery procedures differ from surgeries for medical reasons?
The one main difference is the fact that cosmetic surgery is elective. Once the patient has made the decision to improve him or herself, it causes a positive mental attitude. With this the compliance with all doctor’s recommendations improves, and speed of recovery as well as a successful outcome are statistically greater when the patient is highly motivated.

How long will I need to recover after my surgery?
Recovery times vary depending on several factors. Different procedures will require different recovery times. Then there is the age and health of the patient to be considered as well as the skill of the doctor. An experienced physician can usually provide a fairly accurate estimate based on the above-mentioned criteria during the initial consultation. Recovery time has been dramatically reduced by use of the latest techniques and state-of-the-art equipment.
